Coordinate care from pre-admission to post-discharge for residents on assigned neighborhoods of the facility.
May participate in staff interviewing, selection and hiring process.
Act as a key leader in training, implementations, updates, modifications and ongoing utilization of the electronic medical records.
Perform and document accurate assessment of new admissions.
Identify outcomes and direct staff in implementing nursing care to meet outcomes.
Responsible for discharge and teaching needs for resident and significant others, collaborates with other healthcare agencies in planning discharge needs.
Education and Experience:
Graduate from a two-year associate degree program, three year diploma program or four year baccalaureate degree program.
Successfully complete State Board Exams.
Current license to practice nursing in the State of South Dakota.
Previous experience in geriatric or medical/surgical preferred. Experience in dealing with the public and clients required.
