Avera Marshall Outpatient Therapist-Coordinated Care in Gregory, South Dakota
The Avera Medical Group (AMG) Outpatient Therapist serves as an important component of the clinical care team.
The AMG Coordinated Care Outpatient Therapist is responsible to assess needs of patients facing chronic illness and facilitate plans with patient/family to promote effective care.
Responsibilities are to assist in coordinating, planning, implementing and facilitating programs and counseling services on an outpatient basis. The AMG outpatient therapist’s responsibilities may include several of the following areas: individual, group and family counseling, crisis services/evaluation, tele-therapy services, and current and future groups/programs. All of these areas may involve providing assessments, facilitating treatment and crisis intervention of clientele affected by mental health or dual diagnoses issues. He/she is responsible for the planning, organization and implementation of services to patients and families in the clinic setting, assuring that medically related emotional and social needs are addressed on an individual basis for the patient. He/she will maintain a current knowledge base of available community agencies and resources to facilitate appropriate referrals. Providing community/education services will be a part of this position and some travel is occasionally required.
Certification, Licensure and/or Registration
South Dakota Professional License required; which includes one of the following:
Licensed Clinical Social Worker-LCSW or
Licensed Professional Counselor
Qualified Mental health Provider (QMHP) required within 6 months of employment.
Requires valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier.
