Family Living SpecialistDescription/Job SummaryThe Family Living Specialist, also known as the Shared Living Specialist will oversee a Shared Living Caseload in Sioux Falls and thesurrounding areas. This position monitors documentation, program data, behavior support plans, and all other documentation as assigned. The Family Living Specialist will have a vital role in the Shared Living Leadership team as wellas the Individual Supportteam for the individuals.They willdevelope astrongrapport withShared Living Providers and people supported in order to provide the highest quality supports.Responsibilities/DutiesOversee the quality of programmatic plans, community access, volunteer opportunities, work experiences, and necessary services and supports that are provided to meet the individuals' needs and preferences.Attend and document ISP meetings, and ISP reviews along with CFCM. Monitor annual ISP for progress towards goals and supports for all individuals.Help recruit, onboard,and maintain Shared Living Providers.Complete site visits with the individual present one time per month.Develop, write, present and train restrictive procedure plans and BSPs.Participate in all program audits completed by the Director, or other RHD agency personnel.Ability to be available for rotating "on-call" calendar.Required ExperienceMinimum 1 year experience working with individuals with Intellectual/Developmental Disabilities.Required EducationBachelor's degree in human services field.Required QualificationsBachelors degree in Human Services field. Minimum 1 year experience working with individuals with disabilities. Must be able to pass a background check. Strong written/verbal skills.
